Continuity of care may be disrupted if paramedics see patients with chronic conditions for a minor illness. Part of the relationship building in continuity between a GP and patients is seeing the minor issues as well.

In ‘low integration’ models, paramedics and GPs may have less opportunity to communicate about ‘acute on chronic’ patients.
